feat(database): complete Phase 4.3 PostgreSQL and migration foundation - #39
CRITICAL ARCHITECTURAL SEPARATION INVARIANT:
The internal platform database (dera_platform) is strictly separated from externally inspected customer databases. No Phase 4.3 migration may execute against customer databases, and no customer connection is shared with the platform database. ZERO AI INVARIANT:
Pure deterministic persistence without LLMs, GenAI models, or probabilistic inference.
SQLAlchemy 2.x Base: Base(DeclarativeBase) with explicit, deterministic PostgreSQL naming conventions (POSTGRES_NAMING_CONVENTION) for PKs, FKs, unique constraints, check constraints, and indexes.
Async Engine Factory: create_database_engine with AsyncAdaptedQueuePool, configurable pool parameters (pool_size, max_overflow, pool_timeout, pool_recycle), socket timeouts, and safe lifecycle disposal.
Session & Transaction Lifecycle: create_session_factory and transaction_scope ensuring atomic commits and automatic rollback on failure without connection leaks.
This change adds API response and request contracts, RFC 9457 error handling, and OpenAPI snapshot validation. It also adds internal PostgreSQL persistence, Alembic migrations, database-aware readiness checks, tests, CI workflows, and operational documentation.

krishanth7/dera#38: Defines API contracts and error interfaces used and extended by this change.
Merge Risk: Moderate · previous review 53efc
The transaction-commit and PostgreSQL bind-parameter failures are fixed. Remaining items in this update are small test-cleanup and error-handling hardening. Earlier open concerns from the wider PR still need resolution or explicit acceptance before merge, notably migration-target safety, production TLS enforcement, atomic metadata writes, and the PostgreSQL acceptance evidence.
Security architecture risk: Moderate · up to 53efc
Migration connections do not inherit the configured TLS policy, and transaction handling has ownership and cancellation gaps that can weaken failure containment. Current HTTP routes expose only a bounded connectivity probe, not database writes or migration execution, which limits direct remote exposure.
Retained concerns
Medium · security · inferred: The new online migration runner does not propagate DERA_DB_SSL_MODE, although the runtime engine does and production guidance requires require or verify-full. Configuring verified TLS for the application therefore does not establish the same protection for the DDL-privileged migration connection. A URL-level policy or external enforcement could compensate, but neither is established for deployment.
Medium · reliability · inferred: The first transaction_scope on an already-active transaction commits or rolls back the whole session rather than creating the documented savepoint. When composed inside a caller-owned transaction, this can persist enclosing work before that operation reaches its terminal success state, defeating later rollback. Helper-created nested scopes do use savepoints, and no production consumer was found.
Low · reliability · inferred: Cancellation during the already-active-transaction branch bypasses its except Exception rollback handler while the ownership marker is reset. A directly retained session can consequently remain in a transaction with pending work or locks after the scope exits. Managed-session closure limits this exposure, and the branch using session.begin() delegates failure cleanup to SQLAlchemy.
Security review details
Security Blast Radius
inferred — The migration transport concern is bounded by the selected database and the runner's actual privileges. Documentation recommends DDL-capable migration credentials separate from runtime credentials, so compromised migration transport could have greater authority than ordinary API persistence. Actual production grants and cross-database access are unknown.
Security Findings and Attack Paths
inferred — The supported security concern is an inconsistent transport-policy boundary: a migration invocation can ignore the explicitly selected TLS mode. Interception impact depends on network position, driver or URL behavior, server enforcement, and credential authority. No HTTP path to migration execution or metadata mutation was found in the inspected route graph.
Trust Boundaries and Controls
observed — Readiness uses the startup-created engine and fixed SELECT 1, not request-derived connection details. Platform target selection is configuration authority: settings accept DERA_PLATFORM_DATABASE_URL or DATABASE_URL, and migrations also have a local fallback. The strict customer/platform separation policy is documented, but this target-selection code does not verify database identity; no actual customer-database migration is established.
Resilience and Maintainability Implications
observed — The public connectivity probe has a two-second caller timeout and returns unhealthy on timeout or database exceptions. Managed session wrappers close sessions in finally blocks, reducing retained-resource exposure without repairing transaction ownership or guaranteeing cancellation rollback for directly retained sessions.
Hardening Proposals
proposed — Use a shared transport-policy builder for runtime and migration connections while preserving separate credentials and pool lifecycles. For online migrations, consider requiring an explicit platform-specific target and deployment-enforced database-scoped roles rather than silently falling back. Define whether transaction_scope owns or joins an existing transaction, and make cancellation cleanup follow that ownership contract.

Full details: Linked Issues check
Explanation
Issue #39 is the only active directly linked target. The PR adds the requested async PostgreSQL 16+ and SQLAlchemy 2.x foundation, including AsyncAdaptedQueuePool, typed Base models, request-scoped get_db_session, transaction_scope, the PlatformMetadata model and repository, and async Alembic migrations. It adds the /health/ready database check, credential and error sanitization, local PostgreSQL setup, CI migration verification, operational documentation, and database tests. The supplied implementation summary identifies platform-only configuration through DERA_PLATFORM_DATABASE_URL and no customer connection sharing. It also identifies no AI or inference dependency. The latest commits fix transaction commit behavior, PostgreSQL casts, and Alembic teardown isolation. Test results are author-reported and were not independently executed.
Full details: Out of Scope Changes check
Explanation
The PR range still includes work outside issue #39. Commit 0a208118 adds the Phase 4.2 API contract system, OpenAPI snapshot and governance checks, API documentation and audits, contract tests, and global RFC 9457 error-response changes. These changes are not required for the PostgreSQL persistence foundation. Commit c330598 changes .github/workflows/dependency-review.yml to allow PSF-2.0, but the PR does not identify a database requirement or dependency that needs this change. The database exception subclasses used by the persistence layer are related to #39, but the broader API contract and response-format changes are not.
Resolution
Move the Phase 4.2 API files, tests, documentation, OpenAPI tooling, CI check, and unrelated global error-response changes to a separate PR linked to their own issue. Revert the PSF-2.0 license allowance, or document the specific required dependency and its connection to #39. Update the PR description after the unrelated changes are removed.

Guard __repr__ against unset timestamps.
updated_at is populated by the Python-side default only at flush. On a transient or pending PlatformMetadata, self.updated_at is None. In that case repr() raises AttributeError. This breaks logging, debugger output, and test failure messages for new objects.

Fail fast when no platform database URL is configured.
The function swallows every settings error with except Exception: pass. It then falls back to a hard-coded dera_user:dera_password@localhost URL. In online mode, a missing or invalid DERA_PLATFORM_DATABASE_URL therefore does not fail. alembic upgrade head instead targets whatever listens on localhost:5432 with dev credentials.
For example, an invalid ApiSettings field makes get_settings() raise. The migration then silently connects to the local default. This is the wrong target for a policy that requires controlled, explicit migration execution.
Use the fallback only for offline SQL generation. In online mode, raise a clear error.

Update the database URL resolution around get_settings so settings errors are

tests/integration/test_platform_database.py: 1
51-57: Minor · Data Integrity & Integration · Quick win
Do not run drop_all against a developer's migrated database.
docs/database/TESTING.md tells developers to run this suite with DERA_PLATFORM_DATABASE_URL set to the local dera_platform database. The test calls create_all there. asyncTearDown then calls drop_all. This drops dera_platform_metadata and its data. alembic_version still reports head, so alembic upgrade head will not recreate the table.
Use a dedicated test database or schema. Refuse to run when the database name is not clearly a test database. Alternatively, delete only the rows the test created. Update TESTING.md accordingly.

Remove the generic DATABASE_URL alias for the platform database.
When DERA_PLATFORM_DATABASE_URL is unset, ApiSettings accepts DATABASE_URL. migrations/env.py then uses that value through get_settings(), so migrations can target an unrelated database.
Keep the generic DB_* aliases. They configure connection options and do not select the database target.

Emitted code values fall outside the published ErrorCode enum.
ProblemDetail.code is typed as ErrorCode. The OpenAPI snapshot publishes that enum. These paths return other values:
DeraConfigurationError returns "CONFIG_ERROR".
Any unmapped HTTP status returns f"HTTP_{status}", for example HTTP_422.
Free-form DeraError codes pass through unchanged.
Clients that validate against the schema reject these responses. Constructing ProblemDetail(code=...) from them would also fail.
Add the missing codes to ErrorCode, or map unknown values to the nearest enum member such as INTERNAL_ERROR. Regenerate the snapshot if you add members.

Identifier regexes accept a trailing newline. Use fullmatch.

validate_uuid_str("<valid-uuid>\n")

The validators return the value unchanged, so a newline can reach later SQL or path construction. tests/contracts/test_identifiers.py has no case for this input.

The breaking-change gate compares the live schema with a snapshot that the same PR can update.
The baseline is the committed docs/api/openapi.v1.json in the PR branch. A PR that removes a route and runs --update changes both sides. Then detect_breaking_changes finds nothing and CI passes. OPENAPI_GOVERNANCE.md says breaking changes are "strictly rejected" without an RFC. Only reviewers enforce that.
Take the baseline from the target branch instead. For example, run git show origin/main:docs/api/openapi.v1.json in CI and pass it as a --baseline argument.
Also, detect_breaking_changes iterates every key under a path item. Path-level keys such as parameters hold a list, so base_op.get would raise AttributeError. Restrict the loop to HTTP method names.

This test checks the standard library, not the DERA contract.
json.dumps(..., allow_nan=False) always raises ValueError. The test does not use StrictNumericModel or DeraBaseModel. It still passes if the model configuration changes.
docs/audit/PHASE_4_2_API_ACCEPTANCE.md (REQ-4.2-10) claims that the contract "rejects non-finite floats". API_CONTRACT_STANDARD.md says non-finite values must serialize as null or return a domain error. Assert the model behavior instead.

Fix transaction_scope: it silently loses writes when a prior read has started the transaction.
AsyncSession autobegins a transaction on the first execute(). After that, session.in_transaction() is True. Line 53 then takes the begin_nested() branch. Exiting that block only releases a SAVEPOINT. Nothing commits the outer transaction, and session.close() rolls it back.
Trigger: any get_by_key() or set_value() call followed by transaction_scope(session) on the same session. tests/integration/test_platform_database.py does exactly this in its update step. CI reports expected 'platform_v1_updated' but got 'platform_v1_payload' (line 97). The delete step has the same flaw.
The SQLite in-memory tests do not show this. StaticPool shares one connection across sessions, so uncommitted data is visible to other sessions.
The root cause is that the code cannot distinguish an autobegun transaction from a nested scope. Track scope depth in session.info. The outermost scope owns the commit, even if the transaction was autobegun. Only inner scopes use savepoints.
