feat(auth): complete Phase 4.4 authentication and RBAC foundation - #40
Completes Phase 4.4 — Authentication, Authorization & Role-Based Access Control (RBAC) for DERA.
This milestone establishes DERA's platform identity, authentication, session management, multi-tenant workspace isolation, centralized RBAC, and security audit event logging infrastructure directly in the backend.
In accordance with DERA's core engineering invariants:
Zero AI / LLM Integration: All authentication decisions, token verifications, and RBAC policies are 100% deterministic, explainable, and testable code.
Platform Identity Database Isolation: Platform identity tables (dera_users, dera_user_sessions, dera_workspaces, dera_workspace_memberships, dera_security_audit_events) remain completely separate from external customer database engines.
Least Privilege & Tenant Boundary: Explicit workspace ownership, centralized default-deny RBAC policy engine, and owner protection safeguards.

Related Issues
Part of Milestone 4.4 (Phase 4 — Backend & Platform Infrastructure).
Email Normalization: Conservative email syntax validation preserving plus-addressing (user+dev@example.com) and dot syntax while standardizing domain comparison.
Server-Managed Opaque Sessions: 256-bit CSPRNG hexadecimal tokens stored strictly as one-way SHA-256 digests (token_hash) in dera_user_sessions.
Browser Cookie Security: Dual-transport support with HttpOnly, Secure, SameSite=Lax cookie (dera_session) and Double-Submit CSRF cookie (dera_csrf) with X-CSRF-Token header validation on mutations.
Bearer Token Support: Standard HTTP Authorization: Bearer <session_token> header for programmatic CLI/API access.
Session Revocation: Single logout (POST /api/v1/auth/logout) and global cross-device revocation (POST /api/v1/auth/logout-all). Immediate session denial upon account disablement (dera_users.status = 'disabled').
Abuse Protection: Sliding-window attempt tracking (auth_rate_limiter) with bounded lockout and generic INVALID_CREDENTIALS error responses.
2. Multi-Tenant Workspace & RBAC Engine
Workspace Model:Workspace (dera_workspaces) and WorkspaceMembership (dera_workspace_memberships) with unique user-to-workspace constraints.
Role Hierarchy: Centralized has_permission policy engine supporting Owner, Admin, Editor, and Viewer.
Escalation Safeguards:validate_role_transition blocks non-Owners from promoting to Owner or modifying Owner accounts.
Owner Safeguards:validate_owner_removal prevents ownerless active workspaces.
Tenant Isolation: Reusable require_workspace_permission dependency scoping all resource operations to the verified workspace boundary.
3. Forensic Security Audit Logging
Immutable Events:dera_security_audit_events capturing registration, login/logout, role transitions, and member modifications with request correlation ID tracking and zero credential leakage.
4. Database Migrations
Alembic Revision:002_identity_and_workspaces cleanly upgrading from 001_platform_metadata.
Strict Non-AI Policy: Verified zero generative AI, LLM, or external inference APIs are introduced.
Database Safety: Verified platform identity tables are decoupled from customer database engines.
Credential Protection: Confirmed that no production credentials, passwords, or secrets are committed. Plaintext passwords and session tokens are never persisted or logged.
Least Privilege: Validated that permissions and access boundaries are minimized with default-deny evaluation.

Merge Risk: Moderate · previous review fc29b
The authentication and workspace foundation works, but several edge cases return server errors instead of proper 4xx responses. A concurrent-demotion race can leave a workspace with no owner. Some audit records can be lost, and the published API error contract differs from what the service sends. Fix these before merging, or explicitly accept them.
Security architecture risk: High · up to fc29b
The new identity flow can grant workspace access to an account claiming an unverified email address. The closed-registration setting is not enforced, production database encryption remains optional, and concurrent owner changes can defeat owner protection. Session hashing, revocation checks, and default-deny permissions provide substantial protection but do not resolve these design gaps.
Retained concerns
High · security · inferred: Registration creates an active, authenticated account without proving email ownership. add_member then resolves its target solely by that email and immediately creates an active membership. An attacker can pre-register another person's unused address and receive the role an authorized inviter subsequently assigns to that address, potentially including owner when assigned by an owner. Unique email constraints and role checks do not establish mailbox ownership. Exposure is conditional on that invitation and is limited to workspaces granting membership to the claimed identity.
Medium · security · observed: auth_allow_public_registration advertises a deployment control for disabling self-service registration, but the registration handler never evaluates it and the router is mounted unconditionally. Setting the option to false therefore does not prevent anonymous callers from creating active identities, obtaining sessions, and creating their own workspaces. Existing workspace membership gates still apply.
Medium · security · inferred: Last-owner protection counts active owners before separately updating or deleting a membership, without serializing owner-changing operations. Two owners using distinct sessions can concurrently demote themselves, each observe two owners, and both commit, leaving no owner able to exercise owner-only recovery or delegation. Transactional audit recording and rollback protect individual failures, but not this cross-transaction invariant. The configured engine does not establish serializable isolation, and the membership migration supplies uniqueness and foreign keys rather than an owner-preservation constraint.
Medium · security · observed: The new platform database connection defaults to prefer, accepts weaker modes in production, and forwards prefer to asyncpg. Production validation restricts SQL echo and browser-cookie transport but does not enforce the documented database TLS requirement. A deployment allowing plaintext fallback can expose platform database traffic to a suitably positioned network attacker. This boundary carries identity and authorization data across tenants; actual exploitation depends on network conditions and effective database privileges.
Security review details
Security Blast Radius
inferred — A compromised platform identity inherits that user's active workspace memberships, not unrestricted access to every tenant. The email-preclaim path can accumulate access where inviters independently add the claimed address. A compromised owner identity can additionally administer membership within its workspaces.
inferred — The platform database is a shared security dependency for identities, session validation, membership authority, and audit history. Database transport or credential compromise can consequently have instance-wide consequences, bounded by effective database permissions. The reviewed evidence does not establish access to external customer databases.
Security Findings and Attack Paths
inferred — An anonymous attacker can obtain an active session for a previously unregistered address without mailbox proof. If an authorized workspace administrator later adds that address, the attacker-controlled account receives the assigned membership. The exposed registration switch does not close this path because the handler does not enforce it.
observed — The retained production-TLS finding is supported by a default of prefer and the absence of a production minimum. Prefer is explicitly forwarded to asyncpg. Stronger TLS can be configured, but its availability is not an enforced production invariant.
observed — Two retained password-assurance findings remain relevant to this new authentication boundary: registration accepts a 12-character minimum for email/password-only authentication, and the acceptance record claims Argon2 parameters of 64 MB, three iterations, and four lanes while runtime uses 19 MiB, two iterations, and one lane. The parameter discrepancy establishes assurance drift, not a broken hashing algorithm.
observed — The retained redaction finding concerns a URI-password regex that stops at the first at-sign and can leave a password suffix visible when handling a raw password containing that character. Its counterpart also appears in the configuration fallback. Important counterevidence limits established exposure: normal configuration masking uses parsed URL components, and standard exception mapping returns constant messages rather than driver text. An externally reachable disclosure through those normal paths was not established.
Trust Boundaries and Controls
observed — Browser session cookies default to HttpOnly and SameSite=Lax, and production settings force Secure. Cookie-authenticated mutations use constant-time double-submit CSRF comparison when enabled. Credentialed cross-origin access is restricted by a configured origin allowlist, with wildcard origins prohibited in production.
observed — Logout revokes the resolved session; all-session logout updates only rows belonging to the authenticated user. Newly resolved requests reject revoked or expired sessions and inactive accounts. These controls do not imply cancellation of requests already authorized before a concurrent revocation.
Resilience and Maintainability Implications
observed — Identity/session creation, revocation, and membership mutations record their success audit events within their transaction scopes. Normal exceptions roll back those database changes together, and cookies are issued or cleared after the transaction completes. This provides single-operation failure containment, but does not serialize owner transitions or compensate for a response lost after commit.
Hardening Proposals
proposed — Require verified mailbox ownership or a mailbox-bound invitation acceptance before activating an email-selected membership. Enforce the public-registration setting before creating any identity or session state.
proposed — Serialize all owner-changing operations on a shared workspace lock or an equivalent retry-safe database invariant. Revalidate actor authority and the effective owner set inside that boundary, retaining membership and audit writes in the same transaction.
proposed — Make the production database TLS minimum fail closed, prefer verified server identity where deployment permits, and use structured credential redaction rather than a permissive URI regex. Align password-policy and hashing acceptance statements with measured runtime settings.

Document error responses under application/problem+json, not application/json.
Each {"model": ProblemDetail} entry makes FastAPI put the schema under content: application/json. The committed snapshot shows this, for example docs/api/openapi.v1.json Line 800. At runtime, build_problem_response in apps/api/src/dera_api/core/exceptions.py (Lines 187-191) sends media_type=PROBLEM_JSON_MEDIA_TYPE. Generated clients and contract validators use the response media type to choose a schema. For every 4xx/5xx response, they will find no schema that matches application/problem+json. Declare the content type explicitly, then regenerate the snapshot.
Apply the same content key to every status code. FastAPI also adds the application/json entry from model. Remove that entry with a custom openapi() override if the snapshot must list only one media type.

Keep exc.headers in http_exception_handler.
Starlette's default handler copies exc.headers into the response. This handler calls build_problem_response, and that function has no headers parameter, so the headers are lost. Two cases follow:
On a method mismatch, Starlette raises HTTPException(405, headers={"Allow": ...}). The Allow header is now missing. RFC 9110 requires Allow on a 405 response.
FastAPI security dependencies raise 401 with WWW-Authenticate. Bearer clients no longer get that challenge header.
Add a headers argument to build_problem_response and pass exc.headers.

Return only ErrorCode values in code.
The OpenAPI contract declares ProblemDetail.code as $ref: ErrorCode, which is a closed enum (docs/api/openapi.v1.json Lines 415-418). The handlers can return values outside that enum:
For an HTTPException with 401, 403, 413, 422, or another unmapped status, http_exception_handler returns HTTP_<status>.
Clients that validate responses against the published schema will reject these payloads. Map 401 to AUTHENTICATION_REQUIRED and 403 to PERMISSION_DENIED. Send every other unmapped status to a defined fallback such as MALFORMED_REQUEST for 4xx and INTERNAL_ERROR for 5xx. Also add CONFIG_ERROR to ErrorCode, or map configuration errors to INTERNAL_ERROR.

Make the documented 503 schema match the 503 response body.
The route documents its 503 response as ProblemDetail. When readiness fails, the handler returns HTTP 503 with a ReadinessResponse body (Lines 136-150), and the media type is application/json. Remove claims of checks that detect_breaking_changes does not perform.
The policy says the CI detector rejects removed response properties, changed property types, and narrowed enum values (items 4–6). detect_breaking_changes in scripts/ci/validate_openapi.py (Lines 30-88) checks only removed paths, removed methods, newly required parameters, and removed 2xx codes. It never compares components.schemas. The drift check still fails on such edits, but --update clears it with no breaking-change signal. Reviewers who rely on this document will assume the detector covers these cases. Either add schema-level checks or mark items 4–6 as enforced by manual review.

The example shows values that the implementation does not produce:

Serialize last-owner checks to prevent an ownerless workspace.
count_active_owners is a plain SELECT count(...) with no lock. Take a workspace with two owners, A and B. A demotes B and B demotes A at the same time. Each transaction counts 2 owners and passes validate_owner_removal. Both commits succeed, and the workspace has no owner. remove_member (lines 400-407) has the same pattern.
This breaks the "no ownerless workspace" invariant. No API call can repair it, because only an owner can assign owners. Lock the active owner rows (with_for_update()) or the Workspace row before counting, so concurrent owner mutations run one at a time.

Bound the size of the rate-limiter store.
The class docstring says the limiter is bounded. It is not. record_failure creates a new AttemptRecord for every distinct key. Keys are ip:email, and the email is client-chosen. is_rate_limited and record_failure prune a record only when the same key is seen again. A client that sends failed logins with many distinct emails grows _records without limit. Each such request is cheap, because an unknown user skips Argon2.
The store is also per process. With N workers, the effective limit is N times auth_rate_limit_max_attempts.
Add a periodic sweep of expired keys or a hard cap on key count. For multi-worker deployments, use a shared store. Document the per-process limitation.

Bound the `_records` store in `record_failure` so requests using distinct
client-chosen email keys cannot grow it indefinitely; use a periodic expired-key
sweep or enforce a hard key-count cap. Document that the limiter and its
effective attempt limit are per process, so multiple workers multiply the limit;
use a shared store if multi-worker coordination is required.

Assert revocation directly in test_logout_revokes_session.
A successful logout deletes the cookies. The httpx client then sends no credentials, so /me returns 401 for "no credentials". The test would also pass if revoke_session did nothing.

Correct the "Signed" claim in the CSRF pattern description.
generate_csrf_token returns a random value. verify_csrf only compares the cookie with the header. The token is not signed, and it is not bound to the session. The document names subdomain compromise as a threat. A subdomain attacker can set both the dera_csrf cookie and the header to a value of their choice. The check would pass.
Change the text to "High-Entropy Double-Submit Cookie Pattern". Document that the token is unsigned. Either add HMAC binding to the session token, or state that subdomain cookie injection is out of scope.

register can raise EmailValidationError while normalizing the email and PasswordPolicyError while hashing the password. login can raise EmailValidationError while normalizing the email. Move the duplicate-attempt HTTPException out of transaction_scope.
The auth.register.duplicate_attempt audit event is recorded inside the same transaction_scope that raises the 409. The exception rolls back that scope, so the audit row is discarded. The failed-login path records its event in a separate transaction_scope and raises InvalidCredentialsError only after that scope exits.
Record the audit event in its own scope. Raise the 409 after that scope commits.
