Add whoopsie — “Nuked production at least once.” — for paulinevos, nunomaduro, brendt, and mnapoli.
Changes
Match the four handles exactly and case-insensitively; everyone else sees the badge locked. No bonus XP or stored award is added.
Use the supplied three-path mushroom-cloud SVG, including its 1.7 stroke, in both the profile badge row and achievements dialog. Shell shocked keeps its existing bomb icon.
Cover qualifying, mixed-case, unrelated, and partial-match handles, and document the sixteenth badge in the knowledge base.
Validation
Local checks on the final revision:
bun run build — passed; the base revision was also built for comparison captures.
composer test:lint — passed, with warnings outside the changed files.
Hosted CI is separate from these local results and starts with this PR/branch push.
Visual evidence
Actual local screenshots using the same seeded demo data, not generated mockups. Before captures use base a9b3a1c; after captures use head ebe5985. The demo profile temporarily uses a qualifying handle for the unlocked captures and is restored afterward.
The before column shows the compact row. Each after image combines the compact row with unlocked and locked dialog cards. These images are crops from 1440×900 Chromium captures at DPR 2.

Inspected base and head at 1440×900, 1366×650, and 390×844, light and dark, including unlocked and locked states. The icon renders at 14px with three paths and the supplied 1.7 stroke. No horizontal overflow or browser errors were observed. Buffered layout-shift observations totaled 0 for first paint and opening the dialog. No polling, navigation, keyboard, or motion behavior is changed. Public image links return HTTP 200; published images are pixel-identical to the reviewed captures.
Device verification
Linux Chromium viewport emulation at DPR 2, including a 390×844 narrow viewport; not physical mobile hardware or a native simulator. No changed mobile interaction, input, fixed control, keyboard, or safe-area behavior applies to this static badge/icon addition. Existing dialog scrolling was used to inspect the lower badge rows.
Independent review
A separate read-only reviewer returned CODE: PASS and VISUAL: PASS, with no blockers, for:
Media revision 1: both before images and both after previews shown above, plus the desktop/narrow light/dark inspection captures.
The reviewer inspected the complete diff and actual captures, including the supplied SVG, eligibility/XP coverage, architecture, and layout. The required final-head PHP coverage run completed successfully after that review and before publication; no implementation or visual content changed.
Limitations
No physical-device testing. The checks cover a static badge/icon addition, not new mobile input or navigation behavior. This PR does not deploy or merge the change.

Added the “whoopsie” achievement badge, with a mushroom-cloud icon. It unlocks for Pauline Vos, Nuno Maduro, Brent Roose, and Matthieu Napoli, regardless of handle capitalization, and awards no bonus XP.
Documentation
Updated the achievement documentation to list 16 badges.
The change adds the whoopsie achievement for four case-insensitive handle matches, defines its badge metadata, and adds a mushroom-cloud icon to the achievements dialog and shelf. Tests and documentation now include the sixteenth badge.

Hide the decorative icon from screen readers.
The badge button in achievements-shelf.tsx at Line 320 combines this label with whoopsie. Screen readers therefore announce “Mushroom cloud” as part of the button name. (raw.githubusercontent.com) Make the icon decorative by default.
Based on learnings: decorative icons in buttons should use aria-hidden="true" when the text supplies their meaning.
