Show the skipped warning after an explicit refresh.

When the initial freshness response has no skipped recommendations, triggerAutoRefreshIfStale sets autoRefreshAttempted and shows no warning. The Refresh button starts startRecommendationsRefresh(loadRecommendations), which reloads the recommendations after completion but does not display describeSkippedRecommendations or fetch freshness on the normal completion path. The reload cannot trigger the warning because autoRefreshAttempted blocks the freshness check.

Display the warning from the refresh-completion path, or separate the warning check from the one-time auto-refresh guard.

Review comment at @frontend/src/recommendations.ts around lines 373 - 374:
Update the explicit refresh completion path in startRecommendationsRefresh so it
checks the refreshed freshness state and displays any skipped-recommendations
warning through describeSkippedRecommendations; ensure this check is not
suppressed by triggerAutoRefreshIfStale’s one-time auto-refresh guard.
