Summary

add StepCode app attribution headers to provider requests by default
identify StepCode on OpenRouter with its repository URL, app title, and CLI category
preserve existing provider selection and allow explicitly configured headers to override defaults

Testing

pnpm --filter @step-harness/coding-agent test -- sdk-openrouter-attribution.test.ts
repository pre-commit checks (pnpm check)
