Remove @modelcontextprotocol/sdk dependency for Cloudflare compatibility

Description

This PR removes the dependency on @modelcontextprotocol/sdk which doesn't work on Cloudflare Workers, and replaces it with a custom implementation of the MCP server and transport.

Changes

• Created custom implementation of MCP server in src/mcp/server.ts
• Created custom implementation of StreamableHTTP transport in src/mcp/streamableHttp.ts
• Added type definitions in src/mcp/types.ts
• Updated imports in src/mcp/service.ts and src/routes/mcp.ts to use the custom implementation
• Removed @modelcontextprotocol/sdk dependency from package.json
• Updated documentation in docs/hono-mcp-server.md

Testing

The implementation follows the MCP specification and should work with any MCP client. The custom implementation is designed to be compatible with Cloudflare Workers.
