Add MCP server reference pages for vault tools

Summary

The MCP server exposes six vault tools (listed in the kernel-mcp-server README), but the MCP server reference had no pages for any of them. This adds a Vaults group under MCP Server → Tools:

• manage_vaults: create, list, get, delete vaults; also indexes the other vault tools
• manage_vault_credentials: KERNEL-hosted and 1Password credential paths, create/update specs, connect_account
• manage_vault_items: list/get/invoke/events/delete, fill, webmcp_invoke inputs, 1Password operations
• manage_vault_wallets: Link by Stripe and AgentCard wallet specs, payment_methods
• manage_vault_cards: Link by Stripe and AgentCard card request specs
• manage_vault_provider_configs: organization-owned client configurations

Also:

• adds a Vaults card to the MCP Server overview's Explore grid (same pattern as browser_repl and webmcp)
• adds a "Fill a login from a vault" example to the MCP Server examples page
• adds the create-only vaults parameter to the manage_browsers page

Parameters and behavior come from the tool schemas in kernel-mcp-server/src/lib/mcp/tools/vault-*.ts. Each page carries the preview badge and notes that the tools only show up when vaults are enabled for the org.

Consistency with recent vault changes:

• fill is described as safe to retry after a failed or unknown outcome, matching Describe vault fill as safe to retry #687 and kernel-mcp-server#242. The no-retry guidance for 1pw_fill, webmcp_invoke, and payments is unchanged.
• 1Password access requests (1pw_create_access_request, 1pw_access_request_status) need no browser; only 1pw_fill uses the vault-attached browser. This matches the API change and Document browserless 1Password access requests #686.
