feat(connection): lazy connections - open on first access - #20

ConnectionFactory::mysql()/postgres() no longer connect at creation. PdoConnection::pdo() opens the connection on first use (Lazy Initialization); isConnected() stays false until then. Connection errors surface at first access with the same RuntimeException. An explicit disconnect() is not reopened implicitly. ConnectionPool health checks force the connection on first getWriter()/getReader().
