…ut surfaced Ten new records: the release refusal list drops the open-records refusal when a finding refusal is also raised; a landed pull request can leave the merge queue silently; hand-run review agents have no deadline; the drain has no skip-for-now; a verifier's archive copy inside the worktree reads the live worktree; two flaky tests; two orchestrator lapses (an unread handover overwrite, intent drafts filed without the decomposition table); and an interview question that named an abcd check without saying it was abcd's.
