Fix CLI inspect and scaffold behavior

Summary

• inspect <url> now parses the first discovered feed instead of the last one, restoring the contract documented in README and doc/REQUIREMENTS.md.
• scaffold now creates missing bundled siteinfo and example configuration independently of whether their parent directory (assets, config) already existed, so a partially-scaffolded user directory can be healed by re-running scaffold. Existing siteinfo and example configuration directories are left untouched.

Reason

README, doc/REQUIREMENTS.md, doc/DEPLOYMENT.md and doc/POLICY.md already document this contract, but the implementation had drifted from it: inspect selected the last discovered feed, and scaffold only copied bundled siteinfo/example configuration when it also created their parent directory, leaving partial installs unrecoverable.

No change to public CLI options, subcommands, or arguments. No dependency changes. VERSION remains 26.09.

Added one coherent release item to the v26.09 (Release Date: TBD) section of doc/VERSIONS covering both the inspect and scaffold corrections.
