chore: stop tracking the agent handoff file

.agent/HANDOFF.md is session-local state: a note that gets pasted into a new session. It was committed in #11, which made every later handoff update a repository change that needed a pull request.

This removes the file from the repository and ignores the whole .agent/ directory, which already held the ignored wait state. Nothing else references the file.
