Centralize OS advisory-lock acquisition behind typed state and boundary lock identities so the Codex, OpenCode, and Pi adapters use one implementation while retaining distinct lock files and diagnostics. Update adapter tests and lifecycle documentation to cover process-safe contention, timeout behavior, and the shared lock ownership model. Co-authored-by: SCE <sce@crocoder.dev>
Update mutation-scope tests to match the current state layout and lock behavior by removing stale lock-path checks and the obsolete leftover-lock scenario.
