Document the need for `sce doctor` to surface persisted mutation-scope runtime health instead of exposing only hook registration state. Define the shared status contract, adapter-owned classification investigations, doctor rendering requirements, and validation/context synchronization work while keeping recovery behavior unchanged. Define shared Healthy, Recovering, Blocked, and Invalid statuses plus a per-adapter reason and detail record, and register the new module for upcoming doctor classification work. Expose a read-only health classifier so doctor checks can distinguish healthy, recovering, blocked, and invalid persisted mutation-scope state. Treat unresolved recovery attempts as blocked because the recovery barrier cannot self-clear while attempts remain, and surface state read errors as invalid. Surface Codex recovery-state combinations through the shared health vocabulary so doctor can distinguish normal lifecycle state, self-healing recovery, persistent admission blockage, and invalid persisted state. The read-only classifier is backed by behavioral tests that drive real admission, recovery, and failed-cleanup paths, including repeated global denial for unresolved attempts and orphaned flush reclamation.
