feat: signals pages e apps query no CLI/MCP - #34

Expose GET /api/v1/signals/pages as `cleat signals pages APP` and the MCP tool signals_pages so agents can rank most requested HTTP paths.
Expose POST /api/v1/apps/:id/query as `cleat apps query APP SQL` and the MCP tool apps_query so agents can run read-only SQL on an app.
