fix: MCP apps_create aceita nome do servidor (#21) - #27

Agents pass servers_list names like gestaobem-cx33; the panel only accepts numeric server_id. Cover name lookup, unknown-name listing, and the same resolution on drop register.
Numeric ids still short-circuit so existing create tests keep a single POST. Names and slugs list /api/v1/servers and post the matching id. Unknown values list the known id (name) pairs.
