feat(console): pick account types from cards

Summary

• Replace the new-account provider dropdown with selectable cards, and widen the dialog so each card has more room.
• Keep a long provider list inside a fixed-height scroller so the dialog does not grow with the number of types.
• Give Codex its own blossom mark instead of a letter that collided with Command.

Test plan

• cd frontend && npm run sync
• Open Add account and confirm the cards, Codex mark, wider dialog, and in-list scrolling
