Puts identifiers, parameters, error codes, and package names in inline code across all released versions of CHANGELOG.md, and fixes "Github" in 0.3.0. The unreleased section is left to git-cliff.

The GitHub release notes already match this version. Releases up to 1.8.1 now carry their changelog section. Later releases got the same entry fixes, plus the decoded HTML entities and the v3.4.0 cleanup that #1166 made in the changelog.

Looks good, nothing blocking.

🏭 Apify Factory auto approved ✅ — the machines agreed, which is rarer than it sounds

Every gate passed, and neither the reviewer nor a juror found anything a human needs to look at.
