Adds docs/specs/WORLD_PHASE_C.md, the detailed specification of phase (c), decided with Jon on October 15 and 16, 2026 (WM.36 to WM.59). Cuts the phase (c) design section of WORLD_AND_LANGUAGE.md down to a pointer, adds the new labels to its labels table and a note to its phase (d) section, and fixes three references. Adds the decision rows to DECISIONS.md and the new file to CLAUDE.md reading list. Documentation only; no code changes.
