fix(agent): mark headless guardrail stops incomplete - #1182
Each sets Result.Incomplete = true and a non-empty IncompleteReason. Thresholds (3 empty turns; hint at 2, stop at 6 identical failures), stop answers, message sequence, and retry behavior are unchanged. Interactive runs and --no-completion-gate never take the branch, so their behavior is unchanged.
zero exec already maps Incomplete to exit 4 plus the terminal error/run_end events; these two paths simply never set the flag.
Why
zero exec reported success (exit 0, run_end.status = "success") after both guards halted work, so cron/automation could record an abandoned run as complete. Reproduced on 2e10a4b6 before the change:
After the change, both agent cases are incomplete and the CLI test asserts exit 4, error{code:"incomplete"}, and run_end{status:"incomplete",exitCode:4}.
Tests
TestHeadlessGuardStopsAreIncomplete: both guard paths under the gate; asserts trigger turns (3/6), Incomplete, and a reason mentioning the stop cause.
Existing TestRunStopsAfterConsecutiveEmptyTurns / TestRunStopsAfterRepeatedToolFailures now also pin Incomplete == false with the gate off.
go test ./internal/agent -race -count=1 with the four pre-existing environment failures below skipped: pass
go test ./internal/cli -race -count=1: the new test passes; only the same pre-existing environment failures below
make fmt-check vet: pass
go run ./cmd/zero-release build / smoke: pass
git diff HEAD --check: pass
Seven tests fail identically on a clean upstream/main worktree in this environment (Linux, root, degraded sandbox helper), so they are unrelated to this change:
Headless runs configured to require a completion signal are now marked incomplete when they stop after repeated empty turns or repeated failures of the same tool.
The incomplete result includes a reason identifying the stopping condition and its turn or failure count.
Interactive runs and runs without the completion requirement retain their existing behavior.
The empty-turn and repeated-tool-failure guards returned the run as a
successful final answer even with RequireCompletionSignal enabled, so
zero exec reported exit 0 and a success run_end for work it had just
abandoned.
Set Result.Incomplete and a reason at both stops when the headless gate
is on. Thresholds, stop answers, retry behavior, interactive runs, and
--no-completion-gate are unchanged.

Walkthrough
When the completion signal is required, empty-turn and repeated-tool-failure guard stops now mark the result incomplete and record a reason. Agent and CLI tests cover the guard paths and verify behavior when the completion gate is disabled.

Summary
Mark guarded runs incomplete internal/agent/loop.go, internal/agent/guardrails_test.go
Both guard paths now set Incomplete and record a reason when RequireCompletionSignal is enabled. Agent tests cover headless stops, including empty turns after successful tool work, and confirm interactive runs remain complete.
Verify headless CLI status internal/cli/exec_guard_stop_incomplete_test.go
Tests check the exit code, terminal status, error event, and final text with the completion gate enabled. With the gate disabled, tests expect success and no error event.
Twigpine/zero#1112: It changes daemon handling of exit code 4, which this pull request’s headless guard stops can produce.

Explanation
Issue #1154 requires both guard stops to mark headless runs incomplete and preserve the existing CLI contract. In internal/agent/loop.go, the consecutive-empty-turn and repeated-tool-failure branches set Result.Incomplete and non-empty IncompleteReason when RequireCompletionSignal is enabled. The gate-off path remains unchanged. Tests cover both guards, expected stop turns, reasons, and gate-off behavior. internal/cli/exec_guard_stop_incomplete_test.go verifies exit code 4, error code incomplete, and terminal run_end status incomplete; it also verifies success with --no-completion-gate. The existing thresholds and stop answers remain unchanged.
Full details: Out of Scope Changes check
Explanation
The production changes are limited to the two guard-stop branches required by issue #1154. The agent tests verify headless and gate-off behavior. The CLI test verifies the required terminal contract and uses isolated user state to prevent an existing test flake. The added test tool supports a required progress-then-empty-turn case. No unrelated production behavior or ignored-file changes appear in the whole-PR diff.
Full details: Docstring Coverage
Explanation
Docstring coverage is 40.00% which is insufficient. The required threshold is 80.00%. Docstring coverage is scoped to functions touched by this diff with cyclomatic complexity of at least 2. Documentation-only edits and deletions within surviving functions are included; unchanged diff context and fully deleted functions are excluded. Analyzed 5 functions across 3 files.
Resolution
Write docstrings for the functions missing them to satisfy the coverage threshold.

The headless no-output stop reason used result.Turns, which counts every
turn in the run, so a run with an earlier tool-call turn claimed "stopped
after 4 turns with no output" for a 3-turn empty streak. Use the guard's
emptyTurns counter and say "consecutive".
Pinned by a case with a successful tool turn before the empty streak.
