Scope the counter to each SSR render.

When a long-lived SvelteKit server handles another request, this module-level counter retains increments from earlier renders. SvelteKit documents that server processes can be shared by multiple users. (svelte.dev) On a fresh page load, the browser starts its counter at zero, so matching component order can still produce different server and client formId values. This recreates the hydration mismatch for forms without explicit IDs. Use an ID source scoped to the render that produces the same sequence during hydration.

Review comment at @packages/svelte-form/src/createForm.svelte.ts at line 5:
Replace the module-level _formCounter with an ID source scoped to each render,
so forms without explicit IDs receive the same formId sequence during SSR and
client hydration.
