CCM-15149 sms nudge billing

Description

Change billing view to bill SMS nudge texts to the originator of the App message.

Minor consistency tweaks elsewhere to standardise mapping of columns between standard and originator versions.

Context

Recent confirmation from Sommer that in all cases we should treat an SMS nusge SMS as if it had come from the same source as the App message for billing purposes.
