feat: expand coverage-aware agent integrations

Summary

• expand documented MCP/config support across the major coding-agent clients
• install Scout, Verify, and Auditor profiles with distinct evidence budgets
• require check_index_coverage and source fallback in every tier
• add session, subagent, and post-read context hooks where each client supports them
• hard-enforce restricted Scout/Analysis server allowlists and ownership-safe migration/uninstall
• make Claude Windows hooks portable across Git Bash and PowerShell fallback

Verification

• 719 focused native ASan/UBSan contracts passed
• production C binary build passed
• repository-wide clang-format check passed
• full agent config install/uninstall smoke matrix passed for all 17 profile dialects
• live Scout and Analysis JSON-RPC tool-list checks passed
• Docker was intentionally not run for this change
