Fix misleading LDAP config instructions - #249

Fixes example values for `dt.ldap.username-format`, which uses the `%s` placeholder, not `{0}`. This was always true even in v4, so this mixup was introduced while porting the docs over for v5.
Removes mentions of `dt.ldap.group-filter` and `dt.ldap.user-search-filter`, which are unused, and have been even in v4.
