fix(workflows): port upstream owner and evidence contracts

Summary

Autopilot owners were instructed to babysit their PRs while the PR-opening and babysit playbooks prohibited parts of that lifecycle. This ports pstack's owner exceptions and code-ready verification rounds, with child tracking and isolated replacement writers.

Swarm results now bind evidence to exact SHAs and measurement methods. Decision logs identify run boundaries and preserve corrections. Reflect reviewers no longer need to invent a fixed number of learnings.

Scope

• Update autopilot, babysit, opening-a-pr, and multi-phase-plan together.
• Update swarm, show-me-your-work, and the three reflect reviewer templates.
• Preserve portable model roles, the Node logger, and existing source pins. Reviewed target digests and upstream references accompany the port.

Verification

• npm test: 205 passed, 1 Unix-only test skipped on Windows.
• Both pinned-source baseline checks and strict upstream checks passed.
• npm run check-package and git diff --check passed.
• These checks verify packaging and instruction integrity, not future model behavior.
