fix: restore pymodel release identity after the 0.x rewind

#323 copied upstream 0.x versions and the moonshot-ai VS Code publisher over
the already-published 2.1.0 / pymodel.pythinker line. Put those identities
back, restore the consumed changesets, and drop leftover moonshot-ai branding.
