casing - fields must be camel case

I think restrictions in underlying libs mean that all fields must be in camel case.  at the moment:

- Fields must start with lower case
- Types can start with a cap. 
- Capitalisation of alias is respected

Need to check if there's anyway to change this and if not update examples.
