feat: create SKILLS_INDEX for each repo in workspace

Why

currently if there are skills in a repos referenced by the workspace folder, the skills frontmatter is for those repos are not added to the AI context.

We should generate SKILLS_INDEX.md file for each repo in .allagents.
The AI will decide whether to load the SKILLS_INDEX based on whether it's editing files in a repo.
